High-Fidelity Wraparound (HFW) is a holistic evidence-based care coordination model designed to support youth and families who may be navigating multiple systems such as behavioral health, schools, child welfare, or community services.
Instead of families receiving conflicting guidance from different providers, HFW brings everyone together around one shared plan that is built on the youth’s strengths, goals, culture, and support network.
Through this process, our team works closely with youth, families, providers, schools, and community partners to create a coordinated plan that reflects the youth’s voice and choice while ensuring the right supports are in place.

When Can I Start?
Families and referral partners can begin the process at any time.

You can submit a referral to start services.
Easy Referral Process
- Step 1 — Complete the Referral Form
Submit the referral using the link found below, which directs you to our secure EMR referral portal, and attach the completed referral form. Alternatively, you may email the HFW Referral Form to hfwenrollment@centrosavila.com. For the most efficient processing, we recommend doing both. - Step 2 — Our Team Reviews the Referral
A member of the Centro Sávila team will review the information and contact the family to complete consent forms. - Step 3 — Initial Outreach
We will schedule an introductory conversation to learn more about the youth, family strengths, and current needs. - Step 4 — Wraparound Planning Begins
If the youth is eligible for services, we begin building a wraparound team and individualized support plan.
